*Select the levels where you want employees to receive the survey:
- Select Add if you want the survey to be available to the whole company, OR select the levels 1, 2 or 3 (which appear when selecting Add) to make a more detailed selection.
- Select Remove next to the levels if you wish to delete the added level.
- Select an option(s) from the Employees by department or Employees by job if you wish to select recipients based on these criteria.
- Select email address(es) from the Add more employees that you want to receive the survey… search dropdown list, if you wish to add specific people as recipients OR select File upload to upload an Excel file with recipients’ emails to add them in bulk.
- Select File Upload in Cover Image to upload a thumbnail image for your survey.
*Survey name
- Enter the name of the survey as you wish recipients to see it.
Schedule survey based on a user's employment start date
- This toggles whether the assignee will receive the survey a specific number of days after their employment start date. Note that scheduled surveys like this cannot be set to recur - selecting this will cause the frequency field to be disabled.
Number of days to send survey after employment start date
- Select the period of time that should pass before the survey becomes available. For example you might set up a questionnaire during the induction period, or a regular annual pulse survey.
*Survey start date
- Enter the date from which the survey will be available. If this is in the future, the survey will become available automatically on that date.
*Survey end date
- Enter the last date people can complete the survey.
*Will the survey have conditional blocks?
- Select Yes/No if you wish to set up the survey with different paths based on specific answers. Note: conditional blocks are explained in more detail below.
*Anonymous
- Select Yes/No to indicate whether you’d like the answers to be linked to specific users.
Note: if you set up the survey as anonymous, you can include a personalized message to recipients in different languages in the Customized message for the anonymous survey field below. Otherwise, a standard message will be displayed: "this is an anonymous survey, your answers won’t be linked to your user".
*Do you want to notify employees via push?
- Select Yes/No to indicate whether you’d like employees to receive push notifications about the survey.
*Do you want to notify employees via email?
- Select Yes/No to indicate whether you’d like employees to receive email notifications about the survey.
How many days before the deadline will a reminder be sent?:
- Enter a number of days if you’d like a notification to be sent out to remind employees of the survey deadline.
- Select Next when you finish completing these fields.
Customized message for the anonymous survey
- This customized message is displayed at the top of the survey, if it is not filled in the default message will be displayed.
Add to Homepage Carousel on Mapal One
- If you activate this option, this survey will be added to the homepage carousel.
Note: Only the 4 latest items will be added to the carousel. You can also add an "expiration" date - this will remove the survey from the carousel after the specified period of time.
